Disclaimer and Release of Liability for Energy/Energy Healing Services or Readings
By contacting, making an appointment with and/or receiving readings or services from Ana Quezada and/or accepting an energy session I am agreeing that:
I understand that Ana Quezada is a non-therapeutic and non-medical complementary and alternative health care practitioner and is not licensed. There is currently no license available for the services provided. All services and treatments provided are complementary or alternative to health care services provided by health care practitioners currently licensed by the state of Texas. All services are speculative in nature and we do not guarantee that the results of our work will be satisfactory to the client.
I understand that Ana is an independent and does not work with or for Jan Toomer or Reality Undefined LLC. I agree to release any liability and hold harmless Jan Toomer and Reality Undefined LLC and Reality Undefined website.
I also understand that Ana is:

New Dimension Energy Session™ Practitioner
And that I will pay the fees for Distant or Absent Sessions in advance.
I am at least 18 years of age or have obtained permission from my legal guardian.
I will not be driving or operating heavy machinery during my appointment time.
I understand that Reiki and other energy work/sessions are simple, gentle, hands-on or distant energy techniques that is used for stress reduction and relaxation and that personal experiences with all energy work varies from client to client. I also understand that Reiki and other energy work practitioners do not diagnose conditions or claim to cure specific illnesses. They neither prescribe substances or perform medical treatment, nor interfere with the treatment of a licensed medical professional.
I also understand that Ana Quezada is not a professional, board certified counselors, nor offer any counseling services. It is recommended that I see a licensed medical professional or licensed health care professional for any mental/psychological or physical ailment I, or those in my care, have.
By accepting a Reiki, readings, or other energy work session, I am agreeing that I will not hold Ana Quezada liable and that I release her from any liability and hold her harmless; and that I am solely responsible for any physical, emotional, mental, spiritual and/or psychological issues that may arise, or that I, or anyone else, may perceive to arise – imaginary, or real.
Energy work practitioners reserve the right to:
- refuse any client at anytime
- change prices or services at anytime – providing a 30 day notice
- discontinue services at anytime
Clients:
A client has the right to complete and current information concerning the complementary and alternative health care practitioner’s assessment and recommended complementary and alternative health care services that are to be provided prior to commencement of service(s) for each appointment including: the expected duration of the complementary and alternative health care services to be provided.
Notice regarding client records:
- A client has the right to access their own client’s records and written information therein.
- Client records and transactions are confidential unless the release of these records is authorized in writing by the client or as required by law.
- A client has the right to a coordinated transfer when there will be a change in the provider of the complementary and alternative health care services
